General Position Description:

As a key member of our team at Easter Seals Blake Foundation, the Clinical Care Coordinator will play a vital role in ensuring seamless collaboration between our behavioral health programs and medical programs. This position requires a skilled professional who can assess the behavioral health needs of individuals of all ages and facilitate integrated care coordination.

The ideal candidate will have excellent communication and interpersonal skills, with the ability to work effectively with diverse stakeholders, including members, providers, and community agencies. This role involves assessing member eligibility for services, developing and monitoring service plans, and providing rehabilitation services, including living skills training, counseling, and supported employment services.

The Clinical Care Coordinator will also be responsible for implementing health initiatives and brief interventions to support the health and wellbeing of members. This may involve collaborating with community stakeholders and spending time stationed at the Crisis Response Center to support members in crisis.

Key responsibilities include maintaining EBF productivity, timelines, and quality standards, providing complete documentation and reporting of member behavioral health services, and working in a normal office environment with multiple interruptions. Field positions visiting different sites or homes for member care may also be required.
