Job SummaryWe are seeking a compassionate and skilled Pediatric Home Health Nurse to join our team. As a Pediatric Home Health Nurse, you will be responsible for providing one-on-one care to our patients in the comfort of their own homes. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities- Provide high-quality, patient-centered care to medically fragile children in their homes
- Develop and implement individualized care plans in collaboration with the patient's healthcare team
- Conduct thorough assessments of patients' physical, emotional, and social needs
- Administer medications and treatments as prescribed by the patient's physician
- Monitor patients' progress and adjust care plans as needed
- Communicate effectively with patients, families, and healthcare team members
- Participate in ongoing education and training to stay current with best practices in pediatric home care
- RN/LPN with a current valid license to practice as issued by the State Board of Nursing
- Current CPR certification
- Experience working with pediatric patients, preferably in a home health setting
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
