Job Summary

Centra Care is seeking a skilled Community Health Worker to join our team. As a Community Health Worker, you will play a vital role in promoting health education and wellness in our community. Your unique understanding of the community and language skills will enable you to effectively communicate with diverse members of the community, particularly in our Spanish education partnership with parents, families, and children.

Key Responsibilities
  • Schedule
    • Full-time position with 64 hours every two weeks in the Melrose Community
    • Monday - Friday, 8:00 am - 5:00 pm, with a rotating day off each week
    • No weekends or holidays
  • Pay and Benefits
    • $18.90 or more per hour, with pay determined by experience
    • Full-time benefits, including medical, dental, PTO, retirement, and employee discounts
  • Qualifications
    • Associate's degree or higher
    • Spanish language interpretation required
    • Experience in community outreach, preferably in Spanish-speaking communities, and working knowledge of various healthcare systems
    • Community Health Worker Certification required within 18 months of hire
    • Current American Heart Association Basic Life Support (BLS) card prior to completion of unit orientation
    • Valid Minnesota driver's license, a driving record in good standing, and minimum insurance liability coverage accepted by CentraCare policy
    • Candidate must have reliable transportation and drive their own personal vehicle for this position
