Job SummaryThe Weekend Registered Nurse will be responsible for providing skilled nursing visits, case management, and follow-up care to patients in their homes. You will work closely with our interdisciplinary team to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities- Provide skilled nursing visits, including admission, case management, and follow-up care to patients in their homes.
- Develop and implement individualized care plans that meet the unique needs of each patient.
- Collaborate with our interdisciplinary team to ensure seamless care coordination and communication.
- Confer with physicians to develop and update patient care plans.
- Provide education and support to patients and their families to promote optimal health outcomes.
- Coordinate with other healthcare professionals, including physical therapists, occupational therapists, and home health aides, to ensure comprehensive care.
- Document patient care and progress in a timely and accurate manner.
- Diploma, Associate, or Bachelor's degree in Nursing.
- Minimum of one year of nursing experience, preferably in a home health or acute care setting.
- Current and unrestricted Registered Nurse licensure.
- Current CPR certification.
- Strong organizational and communication skills.
- A valid driver's license and reliable transportation.
