Behavioral Health Therapist

We are seeking a compassionate and skilled Behavioral Health Therapist to join our team at East Liberty Family Health Care Center. As a Behavioral Health Therapist, you will play a vital role in providing high-quality care to our patients, helping them navigate life's challenges and achieve their goals.
Key Responsibilities:- Provide 1:1 psychosocial counseling to patients, addressing their unique needs and concerns.
- Conduct comprehensive psychosocial assessments to identify patients' strengths and areas for growth.
- Develop and implement individualized treatment plans, working collaboratively with patients and other healthcare professionals.
- Coordinate and plan programs and activities to meet patients' social and emotional needs.
- LCSW or LPC credentials required.
- Experience working with patients who have suffered from trauma, abuse, and chemical dependency.
- Experience working with underserved populations and in healthcare settings.
- Bilingual/Spanish speaking a plus.
