Geriatric Rehabilitation Specialist

BAYADA Home Health Care is seeking an experienced Geriatric Rehabilitation Specialist for a part-time or per-diem opportunity performing home health visits in Magnolia, New Jersey.
About the JobThis position involves making home visits to clients in designated geographic territories, conducting diagnostic tests and measurements, implementing protective interventions, and developing individualized care plans.
The ideal candidate will have a current NJ Physical Therapist license, a minimum of one year of recent work experience as a Physical Therapist, and graduation from a program approved by the American Physical Therapy Association.
The salary range for this position is approximately $70,000 - $90,000 per year, depending on experience.
Responsibilities- Conduct home visits to clients in designated territories
- Perform diagnostic tests and measurements, such as mobility assessments and strength evaluations
- Implement protective interventions, including wound care and transfer training
- Develop individualized care plans, incorporating manual therapeutic exercises and gait training
- Educate clients and their families on rehabilitative care and activities necessary for independent living
- Accurately document observations, interventions, and evaluations using a state-of-the-art touch pad tablet
- Current NJ Physical Therapist license
- Minimum of one year of recent work experience as a Physical Therapist
- Graduation from a program approved by the American Physical Therapy Association
- Ability to work independently and manage time effectively
