Job SummaryWe are seeking an experienced and compassionate RN to join our Home Care team. As a Home Care RN, you will provide high-quality patient care in the comfort of their own homes, promoting independence and well-being.
Responsibilities- Provide skilled nursing care to patients in their homes, including wound care, medication management, and education.
- Conduct thorough patient assessments to identify needs and develop personalized care plans.
- Collaborate with patients, families, and healthcare teams to ensure seamless care transitions.
- Document patient progress and outcomes in electronic medical records.
- Current Minnesota RN license.
- Ability to obtain American Heart Association BLS certification within 30 days of hire.
- Valid Minnesota driver's license.
- Previous experience in Home Care and/or Hospice preferred.
