Position Summary:
In this integral role, you will collaborate with a diverse team of healthcare professionals, including advanced practice providers, social workers, and case managers, to ensure comprehensive patient care.
Utilizing state-of-the-art technology, including a robust electronic medical record system and an innovative decision support tool, you will enhance the quality and efficiency of patient care.
Key Duties:

- Perform annual wellness evaluations.
