Responsibilities- Collaborate with the hospital care team to ensure timely completion of lab results, consults, and orders.
- Participate in morning report and census coordination to ensure seamless patient care.
- Actively participate in quality improvement initiatives and review core measure indicators to ensure team goals are met.
- Assess patient needs and collaborate with families to set up follow-up care upon discharge.
- Obtain medical records and assist with medication reconciliation.
- Coordinate care with patients until they have been seen by their primary care provider.
- Graduate of an accredited program for Registered Nurses.
- 3-5 years of nursing experience preferred.
- Current license to practice nursing in the State of Iowa/Illinois for Registered Nurse. BLS certification.
- Ability to use electronic programs related to patient care.
- Strong interpersonal and communication skills.
- Ability to work as a team member and understand guidelines, policies, and procedures.
Exerting up to 50 pounds of force occasionally, and/or up to 20 pounds of force frequently, and/or up to 10 pounds of force constantly to move objects.
