Responsibilities:- Perform a comprehensive assessment of patient and family upon initial visit.
- Develop an advanced plan of care based on assessment data.
- Collaborate with patient and family to achieve improvement and/or stability.
- Manage patient care episodes effectively.
- Prepare timely and accurate clinical notes, orders, and charting.
- Work collaboratively with physicians and agency staff to deliver high-quality care.
- Current and unrestricted RN license.
- At least 1 year of Home Health experience.
- Current BLS/CPR certification.
- Valid Driver's License.
