We are seeking a skilled Weekend RN Home Care Specialist to provide comprehensive care to patients in the comfort of their own homes. As a key member of our team, you will play a vital role in delivering exceptional patient experiences and supporting our mission to build a positive, supportive, and inclusive culture.
Overview
At St. Luke's Health System, our dedicated team of RNs strive to provide compassionate, patient-centered care that meets the unique needs of each individual. Our Home Health services team offers comprehensive health care in the warmth and comfort of the patient's home, from newborn to seniors. We utilize the latest techniques and technology to support people at all ages and stages of life, ensuring seamless transitions between inpatient, home, and outpatient settings.
As a Weekend RN Home Care Specialist, you will:
- Provide comprehensive care to patients with new diagnoses, medications, or recent hospital discharges, requiring respiratory treatment or wound care.
- Ensure patients are progressing with their care plan, providing education related to their disease process, and assessing physical and psychological needs to improve quality of life.
- C coordinate resources and educate patients and their families when it comes time to discharge.
What You Can Expect
- A competitive salary range of $95,000 - $125,000 per year, depending on experience.
- Full-time days (Fridays to Mondays) with 10-hour shifts, including rotating evening call.
- Weekend premium pay, applicable differential/on-call pay, and daily visit requirements below state and national averages.
- Pay for drive time, mileage reimbursement for geographical areas within Treasure Valley (Ada, Boise, Canyon, Gem, Owyhee, Payette, Washington), and sign-on bonus plus relocation assistance available for new hires.
- Cross-training opportunities in hospital-at-home and home infusion.

Benefits and Qualifications
- Education: ASN or BSN degree.
- Experience: 1 year of RN experience; Hospice or Acute Care experience helpful.
- Licenses/Certifications: Current RN License in the State of Practice; Current Basic Life Support (BLS) Certification.
- Valid Driver's License and proof of Auto Insurance.
