This role is perfect for individuals passionate about patient-centered care, mental health, substance abuse, and health behavior. As the primary interface for patients, you'll deliver high-quality services, working under the supervision of our Director of Behavioral Health Program or designee. Collaborate with physicians, case managers, community health workers, mid-levels, primary care providers, or inpatient medical teams to promote efficient use of healthcare services through patient behavioral changes and modifying care delivery systems.

Key Responsibilities:
  • Deliver patient-centered mental health, substance abuse, and health behavior care to assigned patients.
  • Collaborate with healthcare teams to promote qualitative and efficient use of healthcare services.
  • Work under the supervision of the Director of Behavioral Health Program or designee.

Required Skills and Qualifications:
  • Mastery of multiple treatment modalities, including motivational interviewing, cognitive behavioral therapy, behavioral activation, trauma-informed care, and office-based substance use disorder interventions.
  • Strong knowledge of developmental theory, crisis/safety planning, and best practice suicidality assessment and intervention.
  • Education: Master's degree from a program accredited by the Council on Social Work Education or an equivalent organization approved by the Council on Social Work Education.
  • LCPC certification required.
