Clinical Nurse II

We are seeking a skilled Clinical Nurse II to join our team at UNC Health. As a Clinical Nurse II, you will provide high-quality patient care and work collaboratively with our interdisciplinary team to achieve exceptional patient outcomes.
Responsibilities- Provide competent clinical nursing care to patients in a home health setting, utilizing the nursing process to coordinate and deliver care.
- Participate in identifying and meeting learning needs of self and others, attending education programs and using patient education materials to enhance patient care.
- Evaluate patient care and communicate relevant information to promote continuity of care, working collaboratively with the interdisciplinary team.
- Implement care plans, utilizing healthcare organization and nursing standards, policies, and procedures to ensure high-quality patient care.
- Lead by example, developing goals to promote professional growth and minimizing limitations, and participating in development and achievement of unit goals and performance improvement activities.
- Assess and anticipate discharge needs of individual patients and families, developing relationships that promote their ability to advocate for the patient and their own needs.
- Collaborate with patients and families to prepare or update the plan of care, making use of available multidisciplinary resources in planning care.
- Graduation from a school of professional nursing.
- Licensed to practice as a Registered Nurse in the state of North Carolina.
- One (1) year of nursing experience caring for acute and chronically ill patients or an equivalent combination of education and experience.
This position is located at Nash UNC Health Care, with a standard work schedule of 36 hours per week, working nights. The ideal candidate will be a team player, committed to delivering high-quality patient care and working collaboratively with our interdisciplinary team to achieve exceptional patient outcomes.
