About the Role

We are seeking a skilled and compassionate Licensed Therapist PRN to join our team at Perimeter Healthcare. As a Behavioral Health Specialist, you will play a vital role in providing high-quality care to our patients and their families.

Key Responsibilities
  • Conduct psychosocial assessments of new patients and develop individualized treatment plans
  • Maintain contact with family members and significant others to provide education, support, and advocacy
  • Coordinate with community-based social and mental health agencies to ensure seamless care transitions
  • Facilitate group therapy sessions and provide individual counseling to patients
  • Develop and implement discharge plans to ensure patients receive necessary support and resources
Requirements
  • Master's degree in Social Work (MSW) or related field
  • Unrestricted licensure in social work (LMSW or LPC preferred)
  • At least one year of experience in a hospital setting with psychiatric experience
  • Successful completion of CPR and Handle with Care Training within 30 days of employment
