Full Time Medical Care Provider for Seniors

As a skilled Nurse Practitioner (APRN) at Abode Care Partners, you will be responsible for providing high-quality medical care to seniors and patients with complex needs in various settings, including skilled nursing facilities, assisted living, independent living, group homes, and private homes. This role involves examining, treating, and managing chronic and urgent health issues, while collaborating with a multidisciplinary team to ensure comprehensive patient care.

Your Responsibilities:
- Perform evidence-based medical care by planning, directing, and evaluating patient medical needs
- Examine and treat chronic and urgent episodic illnesses, minor injuries, accidents, and other injuries or illnesses
- Order and interpret laboratory and diagnostic testing as needed
- Collaborate with caregivers and other healthcare professionals in providing interdisciplinary care and maintaining a comprehensive plan of care
- Provide referrals to other healthcare practitioners when necessary
- Maintain accurate and up-to-date patient medical records
Requirements:
- Hold an active state Nurse Practitioners license
- Possess current Nurse Practitioner National Board Certification
- Hold active state DEA licensure
- Hold a Master's degree in Nursing (MSN)
- Demonstrate at least five years of nursing experience, with a minimum of two years of nurse practitioner experience preferred
- Demonstrate strong interpersonal communication skills – verbal and written
- Demonstrate excellent time management skills with accountability for practitioner's daily workflow
- Show proficiency in medical instruments and equipment
- Demonstrate a strong knowledge base of HIPAA and OSHA regulations
- Show demonstrated proficiency in computer-based data management programs, electronic medical record (EMR) systems, and Microsoft Office applications
- Possess a valid driver's license with automobile insurance
- Show ability to engage in moderate physical activity
Estimated Salary: $90,000 - $110,000 per annum, based on experience and location.
