We are seeking an experienced Case Manager to join our team at Pinnacle Treatment Centers, where you will have the opportunity to make a meaningful difference in the lives of individuals struggling with addiction.
This full-time position comes with a competitive salary range of $60,000-$80,000 per year, as well as a comprehensive benefits package and opportunities for professional growth and development.
Key Responsibilities- Assess new referrals for appropriate level of care referral.
- Determine need for therapeutic medical, psycho-social, and psychiatric evaluations.
- Coordinate services with counselors and medical case managers.
- Monitor cases by verifying patient attendance, observing and evaluating response to treatment, advocating for needed services, obtaining additional resources, intervening in crises, and providing widespread support.
