Job Overview

We are seeking a skilled nursing professional to join our team at SSM Health. As an RN- Long Term Care, you will provide high-quality care to residents in a long-term care setting.

About the Role

In this role, you will be responsible for providing skilled nursing care to residents according to established care plans and federal/state regulations. You will also oversee the workflow of the unit, administer medications, monitor patient health, and ensure quality end-of-life care while guiding ancillary staff.

Key Responsibilities
  • Provide Quality Care: Provide quality care to acute, subacute, and chronically ill residents, administer medications and treatments according to standards of care, and maintain competency in all required nursing skills.
  • Lead the Team: Provide direction to CNAs and monitor the workflow of the unit to ensure coordination of care. Use the nursing process for assessment, nursing diagnosis, planning, intervention, and evaluation. Report significant changes to physicians as indicated.
  • MEDICATION ADMINISTRATION: Maintain accurate inventory of medications at the beginning and end of each shift. Ensure accurate distribution of medications to all residents according to the 5 rights. Monitor for medication side effects. Ensure that medications are provided in a manner consistent with manufacturer's guidelines.
  • Communication: Give guidance and direction to auxiliary staff. Participate in report meetings at the beginning and end of each shift. Communicate with residents, guardians, and family members on the current condition and needs of residents in accordance with regulations and standards of practice. Provide teaching and training to residents and families as needed.
  • Patient Safety: Ensure that preventative measures are in place such as specialty mattresses, foam boots, weekly skin checks, turning/repositioning schedules, toileting schedules, and cushions in all wheelchairs. Follow up on reports of rashes, open areas, edema, blisters, skin tears, or bleeding.
  • Alarm Response: Respond promptly to alarms. Ensure that restraint alternatives are in place/activated including: personal alarms, cushion alarms, wheelchair belts, tumble mats, alarm mats, bolstered beds, hip protectors.
  • End-of-Life Care: Provide and ensure quality end of life care. Use comfort basket to meet resident and family needs. Ensure hygiene, comfort, and spiritual needs are met. Treat pain/restlessness. Offer family courtesy meal trays and other amenities. Demonstrate compliance with post-mortem care procedures. Participate in departure ceremony.

  • Education: Graduate of an accredited school of nursing
  • Experience: No experience required
  • Licenses/Certifications: Basic Life Support HealthCare Provider (BLS HCP) - American Heart Association (AHA), Registered Nurse (RN) - State of Work Location or Compact Multi-State License
Compensation

This is a full-time position with an estimated salary range of $85,000 - $115,000 per year, depending on qualifications and experience.
