The Registered Nurse provides intermittent skilled nursing services, directing and supervising hospice aide staff in personal patient care. Your responsibilities will include:
- Assessing and evaluating patient needs/problems, identifying mutually agreed-upon goals for care plans with patients.
- Reporting patient status, updates, and discharge planning process to clinical leadership, attending physician, and hospice physician.
- Attending staff meetings and educational in-services as needed.
- Participating in Quality Assessment and Performance Improvement (QAPI) program through submission of data collection.
- Ensuring the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and providing Age-Friendly Care.
- A current unencumbered registered nurse license in the state of practice or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC).
- A valid driver's license and good driving record are required.
- You'll need to demonstrate the ability to work in a field setting and exercise sound nursing judgment.
- Experience assessing patient needs and formulating individualized patient care plans is essential.
- Familiarity with Microsoft Office is necessary for success in this role.
