Job SummaryWe are seeking a skilled Nurse Practitioner or Physician Assistant to join our team as a Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments in patients' homes, identifying potential health risks and developing personalized care plans to improve health outcomes.
Responsibilities- Conduct home visits to assess patients' health and identify potential risks
- Develop and implement personalized care plans to improve health outcomes
- Collaborate with healthcare teams to ensure seamless care coordination
- Provide education and support to patients and their families
- Document patient information and care plans in our electronic health record system
- Masters degree in Nursing or Physician Assistant studies
- Current licensure as a Nurse Practitioner or Physician Assistant in the state of practice
- Board certification in a relevant specialty
- Minimum of 1 year experience as a Nurse Practitioner or Physician Assistant
- Strong communication and interpersonal skills
- Ability to travel to patients' homes and work in a fast-paced environment
