Position Overview:

STGi is actively seeking a dedicated Registered Nurse to deliver exceptional care at our Community Based Outpatient Clinic. This role is pivotal in advocating for Veteran patients across all clinic operations while ensuring seamless coordination with the VA medical center and various stakeholders. A comprehensive understanding of clinical protocols, medical programs, and the interplay of services within the clinic is essential. This position emphasizes customer service excellence and aligns with our commitment to quality care.

Key Responsibilities:

  • Deliver nursing care as per the directives of physicians, mid-level providers, and the Clinic Manager, addressing the physical, mental, and emotional needs of patients.
  • Oversee support staff in the provision of patient care, ensuring adherence to best practices.
  • Engage in PACT team meetings to foster collaborative care.
  • Collaborate with the Primary Care Physician to maintain adequate staffing levels for optimal care delivery and continuity.
  • Implement universal precautions and uphold infection control standards.
  • Supervise medical assistants and licensed practical nurses in gathering initial patient information, including vital signs and symptoms.
  • Prepare patients for examinations and assist providers as needed.
  • Facilitate efficient patient flow in accordance with established access standards and triage protocols.
  • Conduct triage for walk-in patients and manage medical inquiries via phone.
  • Administer medications in compliance with facility protocols and provider prescriptions.
  • Perform CPR and assist in emergency procedures as required.
  • Execute a variety of nursing tasks, including dressing changes, suture removals, and catheterizations.
  • Prepare patients for lab tests by providing necessary information and instructions.
  • Set up and operate essential therapeutic and emergency equipment.
  • Document all relevant patient information accurately in medical records to ensure quality care and continuity.
  • Maintain the clinic reminder tracking system for enrolled patients.
  • Assist in scheduling initial and follow-up appointments based on medical necessity.
  • Keep patients informed about their care plans, including lab results.
  • Provide educational materials and resources to patients and families based on their needs.
  • Support providers in ensuring accurate electronic documentation of patient encounters.
  • Participate in ongoing performance improvement initiatives.
  • Ensure timely and accurate completion of all required clinical and administrative documentation.
  • Contribute to the daily operations of the Community Based Outpatient Clinic.
  • Adhere to all relevant regulations and standards to maintain safety and operational integrity.
  • Uphold confidentiality and protect patient privacy and rights.
  • Assume the responsibilities of the Clinic Manager in their absence, if applicable.
  • Perform additional duties as assigned.

Qualifications:

  • Minimum of one year of experience in a Medical Surgical Unit, Outpatient Facility, or Primary Care setting preferred.
  • Possess a current and unrestricted nursing license in the state of practice.
  • Hold a valid CPR/Basic Life Support certification from an accredited organization.
  • Demonstrate knowledge of professional nursing principles and practices.
  • Exhibit the ability to assess patient conditions and respond effectively in emergencies.
  • Set realistic educational goals for patients based on their needs and understanding.
  • Provide individualized and group health instruction focused on disease prevention.
  • Possess knowledge of pharmacological agents and their implications in patient care.
  • Demonstrate effective communication skills and proficiency in Microsoft Office and Electronic Medical Records.
  • Exhibit strong attention to detail and the ability to manage multiple tasks.

Working Conditions:

  • Moderate physical activity is required, including the ability to handle objects weighing up to 25 pounds and prolonged periods of standing or walking.
  • Work is typically performed in an office environment.
  • Must be prepared to respond to medical emergencies within the clinic.
