We are seeking a highly skilled Registered Nurse to join our team as a Case Manager for Gastrointestinal Conditions at the VA North Texas Health Care System. As a key member of our healthcare team, you will be responsible for assessing, planning, implementing, coordinating, monitoring, and evaluating client care activities to ensure quality and cost-effective care.
Key Responsibilities- Assume leadership role in facilitating care of assigned workload and collaborating to promote positive clinical, financial, and psychosocial outcomes.
- Utilize critical thinking skills and sound judgment to ensure positive outcomes and maximize effective hospital resource utilization.
- Provide leadership for and participate in performance improvement activities, interacting with a wide variety of employees and working independently to accomplish assignments and program goals.
- Support the medical center's mission by possessing and exemplifying the Eight Core Competencies of the High-Performance Model.
- Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth of the United States, or the District of Columbia.
- Graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE).
- 2-3 years of experience in GI lab, GI clinic, case management, or critical care.
This is a full-time position with a Monday-Friday, day shift schedule. May be required to work other shifts, be on call, work weekends and/or holidays as patient care and needs of the service require.
