The Physical Therapist is responsible for developing and executing tailored treatment strategies for patients, adhering to the standards of physical therapy while collaborating with the healthcare team.
KEY RESPONSIBILITIES:
Under the guidance of the Director and the direct oversight of the Manager or Supervisor at the designated treatment facility, the incumbent will carry out the following duties:
Core Responsibilities:
- Assess the physical therapy requirements of referred patients.
- Identify specific issues.
- Create a comprehensive care plan.
- Set measurable short-term and long-term objectives with defined timelines.
- Ensure timely completion of evaluations, re-evaluations, and discharge summaries.
- Manage home exercise programs as appropriate.
- Quickly and accurately assessing patient needs post-evaluation.
- Participating in various meetings, including family discussions, team collaborations, and case management.
- Making recommendations for home care programs.
Assist in the orientation and training of new personnel.
Submit billing for all services, supplies, and adaptive equipment provided to patients.
Comply with Infection Control and Environment of Care Standards.
Support the mission, vision, and values of Catholic Medical Center.
Maintain adherence to Medicare and other insurance regulations as well as institutional policies.
Perform additional related duties as assigned.
KNOWLEDGE, SKILLS, AND ABILITIES:
Candidates must demonstrate the following competencies or be able to illustrate their ability to fulfill the essential functions of the role, with or without reasonable accommodation:
- Deliver patient care tailored to the demographics and specific needs of the patient population served.
- Exhibit proficiency in assessing and adapting treatment to meet patient needs.
- Ability to exert varying levels of force to move objects as required.
- Demonstrate courtesy, compassion, and effective communication with patients, families, visitors, and colleagues.
- Foster a collaborative working environment.
- Accept constructive feedback and take appropriate corrective actions.
- Create a calm and supportive atmosphere for patients and their families.
- Manage periods of high workload or stress effectively.
- Respond appropriately to unexpected situations.
- Knowledge of the APTA Code of Ethics and state licensure laws.
- Willingness to travel to off-site programs and conferences.
- Typically Monday through Friday, with varying schedules by site.
Education:
- Graduate of an accredited Physical Therapy Program with a Bachelor's, Master's, or Doctorate in Physical Therapy.
- Experience in Pelvic Floor Physical Therapy is highly preferred.
- Encouragement of professional memberships and career development.
- Must hold a valid license as a Physical Therapist in the State of New Hampshire.
• American Heart Association Basic Life Support (BLS) certification is required.
