Job Summary

We are seeking a compassionate and skilled Spiritual Care Provider to join our team at Northwestern Medicine. As a Chaplaincy Services Specialist, you will provide spiritual counseling and pastoral care to patients, families, and staff, demonstrating a deep understanding of the cognitive, physical, emotional, and chronological maturation process.

Key Responsibilities
  • Evaluate the spiritual needs of patients and families referred by the Hospice team.
  • Provide home visits to offer spiritual support and counseling services, respecting the wishes and beliefs of the patient and family.
  • Act as a liaison and resource to patients' personal, spiritual, or religious advisors as requested.
  • Collaborate with all disciplines involved in patient care to facilitate the interdisciplinary team approach.
  • Function as a resource for agency staff and participate in the Bereavement Program.
  • Participate in Hospice Interdisciplinary Group meetings, individual care conferences, staff and team meetings, and QI activities as requested.
  • Assist with planning and participate in Hospice Memorial Services and annual Hospice in-service for all staff.
  • Document care and services in accordance with agency policies and procedures.
  • Maintain and enhance professional and managerial expertise through appropriate educational/organizational activities.
  • Attend community meetings, events, and/or workshops as appropriate.
Requirements
  • Excellent verbal and written communication skills.
  • Bachelor's level degree.
  • One unit of ACPE Certified CPE.

Preferred qualifications include a Master's level degree in Theology/Ministry or a related field.
