Job Summary:

VibrantCare Rehabilitation is seeking a highly skilled Physical Therapist to join our team. As a Physical Therapist, you will be responsible for delivering high-quality physical therapy services to patients, caregivers, and facilities in accordance with the principles and practices of physical therapy and VibrantCare policies and procedures.

Key Responsibilities:
  • Develop and implement treatment plans for patients, supervising assistants and aides as needed.
  • Contribute to case management and provide quality patient care.
  • Maintain positive interactions with facilities and clients, enhancing and expanding client relations.
  • Review and adhere to state Practice Act and VibrantCare Clinical Operations Policies & Procedures Manual.
  • Complete required documentation, including pre-certifications, physician reports, daily notes, and charge tickets.
  • Communicate effectively with co-workers, physicians, patients, family members, and third-party payors.
  • Attend and participate in facility meetings as directed by Center Manager.
  • Complete ongoing continuing education coursework to meet state licensure requirements.
  • Maintain current CPR certification.
Requirements:
  • Graduation from a physical therapist education program accredited by the Commission on Accreditation in Physical Therapy Education (CAPTE) or a substantially equivalent education.
  • State licensure/registration where applicable.
  • Ability to work effectively as a team member, with leadership skills sufficient to direct and supervise assistants and aides.
  • Strong organizational, time management, and planning skills.
  • Effective clinical problem-solving skills, including identifying and prioritizing issues, developing and executing sound action plans, and ensuring appropriate results.
  • Strong communication skills for accurate, concise, and organized oral and written presentations of information.
  • Computer skills for effective clinical documentation and management reporting.
Physical Requirements:
  • Ability to lift patients using appropriate biomechanical techniques frequently throughout the workday.
  • Ability to walk, bend, stand, and reach constantly during a workday.
  • Visual acuity sufficient to evaluate, diagnose, and monitor patient needs and maintain accurate records.
  • Ability to speak and hear sufficiently to understand and give directions.
  • Ability to push wheeled equipment through the facility and in the community.
Additional Responsibilities:
  • Utilize time efficiently and effectively to meet productivity requirements.
  • Participate in and present clinical in-services as directed.
  • Maintain current awareness of general changes in the healthcare field and specific issues impacting the practice of physical therapy.
