Job Summary

Northwestern Medicine is seeking a highly skilled and experienced Lead Case Therapist III to join our Community Support Team. This role will serve as a liaison between the CST team and the community, conducting regular quality assurance reviews and Integrated Assessment and Treatment Plans. The ideal candidate will have a Master's degree in social work, psychology, or a related field, and at least 5 years of experience in assessment, individual, family, and group counseling.

Key Responsibilities
  • Conduct regular quality assurance reviews of CST records with the assistance of a Licensed Professional Assistant (LPHA)
  • Conduct Integrated Assessment and Treatment Plans (IATP) and Level of Care Utilization System (LOCUS) assessments in accordance with Rule 132/140 standards
  • Complete clinical chart reviews for assigned CST caseloads or client referrals
  • Manage schedules, appointments, phone calls, paperwork, trainings, meetings, and tracking travel
  • Document patient medical records in an accurate and timely fashion
  • Conduct community-based sessions to support client progress towards identified treatment goals
  • Provide crisis intervention and safety planning, including linkage to other teams or facilities as needed
  • Conduct team meetings, huddles, and supervisions as assigned
  • Provide linkage and referral to internal and external interventions, resources, and services as applicable
  • Track client data on a monthly basis to maintain program fidelity
  • Obtain preauthorization and reauthorization from insurance as needed
  • Maintain Leader on Duty shifts as assigned each month
Qualifications
  • Master's degree in social work, psychology, or a related field
  • At least 5 years of experience and training in assessment, individual, family, and group counseling
  • Master's degree with LSW, LCSW, LPC, or LCPC, or eligible for licensure
  • Preferred qualifications include 5 years in intensive treatment settings and/or mental health settings, CADC, LIC CPC, CERT CADC, LIC LCSW, and LIC MSW
