Home Health Registered Nurse Opportunity

Key Responsibilities:

  • Lead and collaborate with our interdisciplinary team to develop and implement patient care plans
  • Provide skilled nursing care to patients in their homes, promoting independence and self-management
  • Conduct thorough assessments, identify patient needs, and develop individualized care plans
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination

Requirements:

  • RN license in the state you work
  • Graduate from an approved school of practical nursing
  • One year of RN experience and the clinical competence to deliver quality patient care
  • Current driver's license and ability to spend ~20% of your day driving to/from patient locations
  • A commitment to consistently meet critical deadlines for charting
  • The skills needed to self-manage your time and schedule
  • Demonstrated experience with tablets, mobile phones, and EMR software
