Job Summary

We are seeking a highly skilled Registered Nurse to join our team as a Home Health Care Specialist. As a key member of our team, you will be responsible for providing comprehensive nursing care to our patients in their homes, working closely with our interdisciplinary team to develop and implement individualized care plans.

Key Responsibilities
  • Assess patient needs and develop individualized care plans to meet their physical, emotional, and social needs
  • Provide high-quality nursing care to patients in their homes, including wound care, medication management, and education
  • Collaborate with our interdisciplinary team to coordinate care and services, including therapy, social work, and home health aide services
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination
  • Document patient progress and outcomes in accordance with regulatory requirements
Requirements
  • Current, unrestricted state RN license
  • 1+ year experience in a clinical care setting as a nurse
  • Able and willing to travel within branch/office coverage area
  • Must have a dependable vehicle, valid driver's license, and current auto insurance in accordance with state laws
  • Must be able and willing to travel 50%
  • Associates Degree or Bachelor's Degree in Nursing is preferred
  • Previous Home Health or Hospice Experience is preferred
  • Previous experience with HomeCare HomeBase is preferred
