About This Position

We are seeking a compassionate and skilled Pharmacist to join our team at Dignity Health Hospice. As a key member of our interdisciplinary team, you will play a vital role in providing expert medical care, symptom and pain management, and emotional and spiritual support to our patients and their loved ones.

Responsibilities
  • Procure, manufacture, compound, dispense, distribute, store, and administer drugs, biologicals, and chemicals according to standard practices, State and Federal laws, and Departmental policies and procedures.
  • Provide drug information to healthcare professionals, patients, and their families, exercising sound and professional judgment in the performance of job functions.
  • Communicate effectively both orally and in writing, with strong attention to detail and ability to analyze complex tasks, systems, problems, and design effective solutions.
  • Work tactfully and effectively with team members, patients, and their families, and the public, with comprehensive knowledge of practices, principles, methods, and equipment utilized in providing Pharmaceutical services.
  • Organize and prioritize work, with a current and valid California license to practice pharmacy.
Qualifications
  • Minimum Qualifications: One (1) year of acute care hospital pharmacy or home infusion therapy, with a current and valid California license to practice pharmacy.
  • Preferred Qualifications: Pharm.D. Degree, experience in I.V. admixture unit dose distribution, pharmacokinetics dosing, pharmacy intervention, and medication usage evaluation.
