About the Role

As a Care Manager at Sunrise Senior Living, you will have the opportunity to make a meaningful impact on the lives of our residents and their families. We are seeking a compassionate and dedicated individual to join our team in providing high-quality care and services.

Job Summary

The Care Manager is responsible for delivering exceptional care and support to a consistent group of residents and their families in our assisted living and reminiscence neighborhoods. This role involves building meaningful relationships, attending to individual care needs, and collaborating with the community team to ensure seamless care delivery.

Responsibilities
  • Deliver personalized care and services to residents, including assistance with activities of daily living, medication management, and emotional support.
  • Develop and implement Individualized Service Plans (ISPs) that cater to each resident's unique needs and preferences.
  • Collaborate with the community team to ensure effective communication and coordination of care.
  • Participate in quality improvement initiatives and contribute to the development of best practices in senior care.
Qualifications
  • CNA or HHA certification is required; previous experience working with seniors is preferred.
  • Dedication to delivering exceptional customer service and a passion for serving seniors.
  • Ability to work effectively in a fast-paced environment and prioritize tasks efficiently.
  • Strong communication and interpersonal skills, with the ability to build rapport with residents, families, and colleagues.
