Incredible Health is actively hiring a Registered Nurse - Home Health Case Manager to join our team in Lincoln, MA.
Compensation and Benefits:The salary for this position ranges from $75,000 to $150,000 per year. Additionally, we offer a comprehensive benefits package, including healthcare coverage, 401K, paid time off, and tuition assistance.
Job Description:This role involves providing skilled nursing care to patients in their homes, developing personalized care plans, and educating patients and families on various aspects of healthcare. The ideal candidate will have experience in areas such as catheter care, clinical pathways, skin assessment, and utilization review.
We are seeking a dedicated and compassionate RN with a BS/BSN degree or higher from an accredited school of nursing and an active, unencumbered license in the state of Massachusetts.
