Job Title: RN Case Manager - Hospice Care Specialist

We are seeking a compassionate and skilled Registered Nurse to join our team as a Hospice Case Manager. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality, patient-centered care to individuals and their families.

Responsibilities:
  • Assess patient needs and develop personalized care plans in collaboration with the clinical team.
  • Provide emotional support and education to patients and their families, empowering them to make informed decisions about their care.
  • Collaborate with healthcare providers to ensure seamless transitions of care and optimal patient outcomes.
  • Participate in interdisciplinary team meetings to discuss patient progress and develop care plans.
  • Document patient information and care plans in a timely and accurate manner.
Requirements:
  • Current RN licensure in the state of Arizona.
  • Minimum one year of experience as a Registered Nurse in a medical-surgical or acute care setting.
  • Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
  • Computer proficiency, including experience with electronic medical records (EMRs).
  • CPR certification.
