We are seeking a skilled and compassionate Advanced Practice Clinician to join our team at a Skilled Nursing Facility. As a Nurse Practitioner or Physician Assistant, you will be responsible for managing a panel of residents, focusing on treating patients in place in collaboration with a full primary care team.
Key Responsibilities:
• Conduct comprehensive assessments and develop individualized plans of care
• Attend care team meetings and communicate with family members/caregivers
• Ensure the care team understands patients' goals and needs of care
• Collaborate with physicians, care coordinators, facility staff, and other healthcare professionals
Requirements:
• Texas NP or PA license
• DEA
• 2+ years advanced practice experience
• 2+ years geriatric care experience (as APP)
