About the Role

We are seeking a compassionate and skilled Registered Nurse to join our Hospice team as a RN Case Manager. As a Hospice RN, you will be responsible for providing high-quality patient care and support to individuals and their families in need of end-of-life care.

Key Responsibilities

Provide direct patient care services to hospice patients and supportive care to the patient's family

Act as primary liaison between physician, patient/family and hospice team

Evaluate patient and family needs and assist in the preparation and execution of the plan of care for each patient

Maintain all assigned patient clinical records

Requirements

RN license in the state of requested employment (or eligibility to obtain)

1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology strongly preferred

Current driver's license and automobile insurance

Current CPR certification
