Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our team at Eden Health. As a Home Health Registered Nurse, you will provide high-quality patient care and support to individuals in their homes throughout King county.

Responsibilities
  • Provide admission, case management, and follow-up skilled nursing visits for Home Health patients
  • Administer ongoing care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager
  • Implement patient care plans in conjunction with patients and families to assist them in achieving optimal resolution of needs/problems
  • Coordinate and oversee the work of Home Health Aides and provide written instructions that reflect current plans of care
  • Discharge patients after consultation with the physician and Clinical Manager, preparing and completing needed clinical documentation
Requirements
  • Graduate of an accredited nursing program
  • Current licensed Registered Nurse (RN) in the state of practice
  • Experience as an RN with one of the following preferred: one year of experience in an acute-care setting within the last 24 months, six months of Home Health experience within the last 12 months
  • Credentials in specialty areas preferred (wound care, OASIS certification, IV, ET)
  • Valid driver's license and reliable transportation
  • Proof of automobile insurance (MVR checked)
  • Proof of current CPR certification
  • Able to pass criminal background check and national sex offender clearance
