We are seeking a highly skilled and experienced Physical Therapist to join our team as a Home Health Clinical Supervisor. As a Clinical Supervisor, you will be responsible for overseeing the delivery of high-quality patient care in the comfort of their own homes.
Key Responsibilities:- Assure ongoing interdisciplinary assessment of patients to ensure comprehensive care.
- Supervise paraprofessional and licensed professional team members to ensure adherence to clinical standards and regulations.
- Use clinical decision-making to efficiently manage patient populations and promote patient progress towards established goals.
- Develop and implement education programs for patients, families, and caregivers to promote patient progress and independence.
- Graduate of an accredited Physical Therapist program and active PT license in Colorado.
- Minimum of three years of physical therapy experience in home healthcare, with two years of management experience preferred.
- CPR certification and knowledge of state and federal home health regulations.
- Clinical proficiency in nursing specialty, strong communication skills, and compassion.
