We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our Home Health team in Topeka. As a Home Health LPN, you will provide high-quality patient care in the comfort of their own homes, working closely with a multidisciplinary team to deliver exceptional outcomes.
Key Responsibilities:- Provide home-based care to patients with illnesses, injuries, and chronic diseases, under the supervision of an RN.
- Work collaboratively with physicians, RNs, CNAs, and aides to deliver quality patient care.
- Take patient vitals, conduct assessments, document progress, and report changes to the RN supervisor.
- Assist the RN in carrying out the patient's plan of care and teaching patient self-care techniques.
- Educate family members on the patient's plan of care, medication, and home safety.
- Collaborate with the interdisciplinary team to coordinate patient care and plan for discharge.
- Graduate of an accredited practical nursing program and active LPN license in Kansas.
- CPR certification.
- Knowledge of state and federal home health regulations.
- Clinical proficiency in nursing, strong communication skills, and compassion.
