The RN Patient Flow Coordinator is responsible for coordination and communication of transfer requests for LCMC Health Adult Hospital Market (AHM) facilities by utilizing problem-solving, critical thinking, and excellent customer service skills. This professional will act as a facilitator and decision-maker while applying nursing knowledge as a foundation for placement decisions and communications. This role will also be responsible for bed planning and throughput facilitation at designated LCMC Health Adult Hospitals.
This job involves serving as the point person to facilitate incoming transfers/transports, maintaining communication with all parties including but not limited to attending/accepting MD and referring facility. Uses problem-solving skills to determine the best course of action which will be satisfactory to the patient and referral health care team and optimize communication with the appropriate physician. Utilizes nursing experience and education as foundations for gathering accurate clinical data to assure appropriate and prompt transfer and placement of the patient.
