About the Facility - Jicarilla Service Unit
The Jicarilla Service Unit, also recognized as the Jicarilla Apache Health Care Facility, is dedicated to serving the members of the Jicarilla Apache Nation, with a patient population exceeding 4,000. The clinic, known in the Jicarilla Apache language as Nzhonachi'idle'ee, meaning "a place to get well," is accredited by the Accreditation Association for Ambulatory Health Care, Inc. (AAHC). As a Patient-Centered Medical Home and Certified Dental Home, the facility boasts a committed team collaborating with local community programs to emphasize health promotion and disease prevention.
Key Responsibilities
- Lead initiatives for weight management and exercise programs as necessary.
- Conduct comprehensive nutritional assessments and gather relevant dietary histories.
- Formulate care plans for patients identified as at nutritional risk.
- Document clinical information in electronic medical records.
- Offer nutritional consultations via phone or home visits as required.
- Provide therapeutic diet counseling for patients referred by physicians.
- Engage in multidisciplinary team meetings for the management of patients with chronic conditions.
- Deliver nutritional consultation services to community programs upon request, as clinic scheduling permits.
- Facilitate in-service training for client staff as needed.
- Adhere to all Client Privacy Act, HIPAA, and ethical standards.
- Prepare and submit monthly narrative reports to the Director of Public Health Nursing.
- Possession of a current nutrition license in New Mexico.
- Registered with the American Dietetic Association as a Registered Dietitian Nutritionist.
- Degree or diploma from an accredited professional nutrition program.
- Certification as a Diabetes Educator.
- Current Basic Life Support (BLS) certification is required.
