Job Summary: We are seeking an experienced Licensed Practical Nurse (LPN) to join our team as a Wellness Nurse. The successful candidate will be responsible for conducting health assessments, creating personalized care plans, and administering medications and treatments according to physician orders and established protocols.

- Educate residents and their families on health-related topics, promoting preventive care and empowering them to make informed decisions about their health.
- Respond to medical emergencies with calm professionalism, providing immediate and appropriate interventions to ensure residents' safety and well-being.
- Maintain accurate and up-to-date resident medical records, adhering to confidentiality and data protection policies.
- Foster a supportive and compassionate environment for residents and their families, promoting open communication and transparency throughout their care journey.
- Participate in ongoing training and professional development opportunities, staying up-to-date with best practices and advancements in geriatric nursing.
- Degree or future graduate from an accredited nursing program.
- Current State License as a Licensed Practical Nurse.
- Excellent communication skills, both verbal and written, with the ability to engage effectively with residents, families, and healthcare teams.
- A minimum of 1 year of experience as an LPN in a long-term care setting, with a proven track record of delivering high-quality care and achieving positive outcomes.
- Natural problem-solving skills, with the ability to think critically and make sound judgments in complex situations.
- Commitment to interacting professionally and with compassion toward residents, families, and other staff members.
- Competitive salary, commensurate with experience and qualifications.
- Shift Differentials: $1.50 for PMs and NOCs, and $1.50 weekend differential on all shifts.
