About the Role:
As a Home Health Physical Therapist at CenterWell Home Health, you will play a vital role in delivering high-quality, patient-centered care to individuals in the comfort of their own homes. Our team is dedicated to helping patients recover from illness, injury, or surgery, and regain their independence and quality of life.
Key Responsibilities:
- Develop and implement individualized physical therapy treatment plans to address patients' specific needs and goals.
- Conduct thorough assessments and evaluations to identify patients' strengths and limitations, and develop targeted interventions to improve their functional abilities.
- Collaborate with patients, families, and healthcare teams to ensure seamless care coordination and communication.
- Provide education and training to patients and caregivers on proper techniques and strategies for managing chronic conditions and promoting overall health and wellness.
- Document patient progress and outcomes in a timely and accurate manner, and participate in quality improvement initiatives to enhance patient care and outcomes.
Requirements:
- Master's degree in Physical Therapy from an accredited program.
- Current and unrestricted Physical Therapy license in the state of practice.
- Minimum of one year of experience in home health or similar setting.
- Strong communication and interpersonal skills, with ability to work effectively with patients, families, and healthcare teams.
- Ability to lift, bend, and move patients as needed, with or without reasonable accommodation.
