Job Description

BHSN, a leading provider of whole person care, is seeking a skilled Rehabilitation Practitioner to join our rapidly growing team. As a key member of our Adult Home and Community-Based Services (HCBS)/Community Oriented Recovery & Empowerment (CORE) team, you will have the opportunity to make a meaningful difference in the lives of individuals with mental health and substance use disorders.

Your Role:

As a Rehabilitation Practitioner, you will provide home and community-based individual and group services to an assigned caseload, including psychosocial rehabilitation, habilitation, family support and training, education support services, pre-vocational services, intensive supported employment, and/or ongoing supported employment. You will work closely with service recipients, their support networks, and other healthcare professionals to develop and implement individualized service plans that promote recovery and independence.

Responsibilities:

  • Provide engaging interventions for individual recipients in accordance with productivity standards
  • Participate in the development, writing, and timely completion of individual service plans, progress notes, and other required documentation
  • Coordinate and collaborate with service recipients' support networks
  • Travel to meeting sites in the community and provide transportation to recipients when working on community-based goals
  • Attend department meetings and trainings as required

Requirements:

  • Bachelor's degree or equivalent experience working with individuals with mental health and substance use disorders
  • Experience in or knowledge of the mental health/addiction field preferred
  • Ability to problem solve and think creatively
  • Valid NYS driver's license with a driving record acceptable to BHSN insurance carrier and reliable transportation required for local travel
  • Strong interpersonal, communication, and organizational skills
  • Proficient in Microsoft Office software
