Job Title: RN Care Specialist

We are seeking a skilled and compassionate RN Care Specialist to join our team at Saint John's On The Lake. As a key member of our healthcare team, you will provide high-quality patient care and support to our residents.

Responsibilities:
  • Provide comprehensive nursing care to residents, including assessments, care planning, and implementation of individualized plans of care.
  • Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
  • Supervise and delegate tasks to LPNs, Med Passers, and RAs to ensure efficient and effective care delivery.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures.
  • Assume command in emergency situations and ensure the safety and well-being of residents and staff.
Requirements:
  • Graduate of an accredited School of Nursing and currently licensed as a Registered Nurse in the State of Wisconsin.
  • At least 2 years of experience in a geriatric/rehabilitation setting, with one year of acute medical/surgical hospital experience preferred.
  • CPR certification and ability to read, speak, and write English.
  • Familiarity with State and Federal regulations/guidelines for primary work area, including Skilled Nursing Facility (Windsor), CBRF (Stratford and Canterbury), and RCAC (Towers).
  • Basic computer skills with proficiency in Microsoft Outlook and ability to provide constructive performance feedback.
Preferred Qualifications:
  • Experience with the MDS system and ability to complete the nursing component.
  • Ability to work flexible hours and adapt to changing organizational needs.
  • Excellent written and verbal communication skills, with a warm and cheerful demeanor when interacting with residents, families, and staff.
