Responsibilities- Assessment and Care Planning: Conduct thorough assessments of patients' physiologic and psychosocial health status, identifying areas for improvement and developing individualized care plans.
- Collaboration and Communication: Work closely with physicians, nurses, and other healthcare professionals to coordinate patient care, share knowledge, and ensure seamless transitions.
- Medication Administration and Education: Safely administer medications, provide education to patients and families, and promote healthy behaviors.
- Documentation and Reporting: Maintain accurate and detailed records of patient interactions, treatments, and outcomes, using standardized documentation tools and reporting systems.
- Professional Development: Stay current with best practices, attend educational workshops, and participate in quality improvement initiatives to enhance patient care and outcomes.
- Graduate from an accredited School of Nursing with a Bachelor's Degree in Nursing (BSN) or higher.
- Minimum 1-2 years of professional nursing experience in gerontology or a related specialty.
- Current license to practice as a Registered Professional Nurse in New York State.
- BLS certification and specialized certifications as needed.
