- Provide comprehensive patient assessments, collect health data, and communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions.
- Deliver patient education and support, addressing cultural and learning barriers to promote patient self-management and empowerment.
- Participate in performance improvement activities, nursing research, and professional development initiatives to enhance clinical expertise and contribute to a culture of excellence.
- Collaborate with technical staff to provide safe and effective care, including medication administration, wound/ostomy/skin care, and other specialized services.
- Current professional nursing licensure in the State of IL required.
- Graduate of a NLN accredited school of nursing required; BSN strongly preferred.
- BLS/CPR Certification for the Healthcare Provider required.
- Minimum of 2 years Maternal Child Health experience strongly preferred.
