    • Liaise with insurance companies, pharmacies, and other stakeholders to resolve any obstacles or challenges that may arise, ensuring uninterrupted access to advanced diabetes technology.
    • Utilize a comprehensive toolkit to troubleshoot issues and provide timely support to patients, ensuring they receive the best possible care.
  • Communication and Coordination
    • Facilitate effective communication among healthcare teams, patients, and external stakeholders to ensure timely access and utilization of advanced diabetes technology.
    • Coordinate troubleshooting efforts and training sessions with healthcare teams and company representatives, utilizing a standardized toolkit to ensure patients receive optimal support.
    • Ensure patients have access to all necessary resources and education to comprehend the initiation and utilization processes related to advanced diabetes technology.
    • Reinforce education and training to encourage and sustain optimal use of diabetes technology among patients.
    • Maintain a current understanding of patients' challenges and provide personalized support tailored to their specific needs.
  • Confidentiality and Compliance
    • Adhere to strict confidentiality guidelines and safeguard patient information and sensitive data throughout all interactions and processes.
  • Additional Responsibilities
    • Schedule and conduct video visits with patients requiring additional assistance, ensuring their needs are addressed effectively.
  • Organizational Duties
    • Document all patient encounters and ensure compliance with reporting standards set by the organization in the electronic medical record.
    • Maintain confidential records of patient information, progression, and forms.

Requirements
  • Education and Experience
    • High school diploma or equivalent required; additional education may substitute for required experience.
    • Two years of related experience preferred.
  • Skills and Abilities
    • Familiarity with advanced diabetes technology and willingness to become facile with its use.
    • Personal connection with diabetes and genuine interest in working with patients with diabetes.
