The home health aide works under a registered nurse or therapist, performing personal care services to meet patients' needs. They observe patients, report observations, and document care performed. The aide will be assigned by a Registered Nurse to promote quality, continuity, and safety of patient care.

Key Responsibilities:

  • Assisting patients with toileting activities.
  • Planning and preparing nutritious meals.
  • Assisting with feeding, if necessary.
  • Taking and recording temperatures, pulse, respiration, and blood pressure.
  • Assisting with ambulation and exercise according to the plan of care.
  • Performing range of motion and other simple procedures as ordered.
  • Assisting with medication administration.
  • Doing patients' laundry, as appropriate.
  • Meeting patients' safety needs and using equipment safely.
  • Reporting on patients' condition and significant changes to the assigned nurse.
  • Adhering to documentation and care procedures and standards of personal and professional conduct.
  • Meeting attendance and reliability guidelines to ensure quality and consistent patient care.

Qualifications:

  • Meets training requirements and successfully completed a training and competency evaluation program or a competency evaluation that meets state requirements.
  • Registered on the Home Care Aide Registry through the Department of Social Services.
  • At least 18 years old.
  • Ability to read and follow written instructions and document care given.
  • Self-directing with the ability to work with little direct supervision.
  • Empathy for the needs of the ill, injured, frail, and impaired.
  • Current CPR certification.
  • Tact, patience, and good personal hygiene.
  • Licensed driver with an insured automobile in good working order.
