Anova Hospice & Palliative is seeking a skilled and compassionate Registered Nurse to join our team as a Hospice Nurse - Patient Care Specialist. In this role, you will provide high-quality patient care and support to individuals and their families in their homes or designated facilities.
Key Responsibilities- Conduct comprehensive assessments of patients and families to determine their needs and develop individualized care plans.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Provide education and support to patients and families on disease management, symptom control, and end-of-life care.
- Administer medications and treatments as prescribed by physicians and maintain accurate records.
- Participate in on-call duties and respond to patient needs in a timely and compassionate manner.
- Current RN licensure in the State of Pennsylvania.
- At least 3 years of experience as a Registered Nurse, with a minimum of 1 year in hospice care.
- Strong organizational and communication skills, with the ability to work effectively in a team environment.
- Reliable transportation and a valid driver's license.
