Job Description

We are seeking a full-time, Nurse Practitioner to join our high-risk home-based care team. As a member of our team, you will be part of a compassionate community of healthcare professionals who work hard every day to make healthcare better for all.

Key Responsibilities
  • Provide high-quality, patient-centered care to patients in their homes
  • Work autonomously to diagnose and treat patients with acute, chronic, and potential health problems
  • Develop and implement comprehensive care plans to meet the unique needs of each patient
  • Collaborate with other healthcare professionals to ensure seamless care coordination
  • Stay up-to-date with the latest medical research and best practices to provide evidence-based care
Requirements
  • Graduate of an accredited Nurse Practitioner program
  • Two or more years of in-home primary care clinical experience as a Nurse Practitioner
  • Current, unrestricted State license or eligibility for licensure
  • Valid drivers license
  • Active Drug Enforcement Agency (DEA) registration or eligibility for registration
  • Active Medicare/Medicaid Enrollment or eligibility to enroll
  • Certified in Basic Life Support (BLS) or Advanced Cardiac Life Support (ACLS)
