Job Summary

Mass General Brigham Healthcare at Home is seeking a skilled Registered Nurse (RN) to provide high-quality clinical services to patients in various settings within the community. As a member of our interdisciplinary team, you will deliver patient care that aligns with our organization's philosophy, policy, and standards of community health practice.

Responsibilities
  • Provide skilled nursing care to patients in various settings, including assessment, teaching, treatment, documentation, and care coordination.
  • Perform admissions process and case management responsibilities in accordance with MGB Healthcare at Home policies and standards of practice.
  • Utilize thorough and timely electronic documentation and interdisciplinary communications to ensure seamless care delivery.
  • Collaborate with healthcare providers to develop and implement individualized care plans that meet patients' unique needs.
  • Conduct thorough assessments, develop and implement care plans, and provide education to patients and their families to promote optimal health outcomes.
Requirements
  • Graduate of an approved School of Nursing.
  • Current Massachusetts license as Registered Nurse.
  • Minimum of one year previous nursing experience in an acute care setting, with prior home care experience preferred.
  • Ability to work with various computer software and knowledge of hand-held patient care computer devices preferred.
  • Strong interpersonal and customer service skills, with the ability to work with a diverse population.
  • Ability to communicate effectively in writing, verbally, and electronically.
  • Ability to work as a member of an interdisciplinary team of healthcare providers.
  • Ability to organize and prioritize work, and adapt to changing situations.
  • Ability to work independently, be self-directed, and adapt to unpredictable circumstances.
