Job Summary

As a key member of our team, you will provide clinical leadership and direction to a team of Mental Health Professionals, delivering comprehensive community-based mental health services and maintaining accurate clinical documentation.

Key Responsibilities
  • Clinical Leadership: Coordinate the multi-disciplinary process on behalf of the individual consumer, assign referrals, review clinical activities, and certify diagnosis and treatment plans.
  • Care Coordination: Ensure clients receive clinically sound triage/referral and ongoing services, perform diagnostic assessments, and plan treatment modalities.
  • Clinical Excellence: Maintain accurate clinical documentation, integrate ethical principles, evidence-based practice, and research findings into all areas of practice.
Qualifications
  • Education: Master's or doctoral degree in counseling and guidance, rehabilitation counseling, social work, psychology, pastoral counseling, family therapy, or a related field.
  • License/Certification: Valid Illinois driver's license, Basic Life Support (BLS) certification, Crisis Prevention Institute (CPI) training, and certification in Illinois Medicaid Comprehensive Needs & Strengths (IM+CANS).
